Oak Ridge Track Club --Entry Form & Waiver  

I agree to participate the event listed below:

I hereby waive/disclaim/forego all legal actions and claims of myself and my heirs against Oak Ridge High School (ORHS), Oak Ridge Track Club (ORTC), and any related officials of this race\event for any illness or injury which may directly or indirectly result from participation in the ORTC race event.  I furthermore hold harmless both ORHS & ORTC for any thefts or vandalism to my personal property while on the premises.  I further state that I have trained and I am in proper physical condition to participate in the above listed events. 

This entry is invalid unless entrance fee is paid and signed by entrant (if over 18 years of age) or parent/guardian if under 18 years of age.
